Knowing your health is bad can start with self assessment on a regular. You need to pay attention to your overall body function. Sleep pattern, sluggishness, change in skin, sudden weakness, check your blood pressure, glucose, BmI, bowel movement and urine. You should do physical exams and check up with primary care physician. When you always feel weak or when you have irregular vitals like high BP, then you know that your health is not good. You also get to know when you do periodic tests

Symptoms of ill health are lack of energy or when you are frequently sick, difficulty sleeping or waking. Check the vital parameters like glucose or BMI on a regular basis and consult a doctor.

If you're feeling off you should go see your doctor. They can check your vitals and get bloodwork done to see if there's anything bad going on. If you have issues sleeping you should seek a medical professional. Sometimes having sleep apnea can cause other issues such as high blood pressure.
